1. Grant of Permission

I, ("Participant"), hereby grant ("Facility") and its authorized representatives permission to capture, use, and publish photographs, video recordings, and audio recordings of me taken during classes, events, competitions, and general use of the facility.

2. Permitted Uses

I understand that the images and recordings may be used for the following purposes:

Website and social media content
Marketing and advertising materials (print and digital)
Promotional videos and highlight reels
Press releases and news features
Internal training and educational materials

3. No Compensation

I understand and agree that no compensation will be paid to me for the use of my image, likeness, or voice, now or in the future.

4. Waiver of Rights

I waive any right to inspect or approve the finished product or the use to which it may be applied. I release the Facility from any claims arising from distortion, alteration, or use in composite form.

5. Duration

This release is valid indefinitely unless I revoke it in writing. I understand that revoking this release will not affect materials already published or distributed.

6. Minors

If the participant is under 18, a parent or legal guardian must sign below.
